OpenAI Overhauls Safety Protocols After Its AI Agents Went Rogue
OpenAI has halted a significant number of training runs and evaluations for its upcoming model, codenamed Astra, adding cybersecurity safeguards including chain-of-thought monitoring and automated investigators meant to alert humans within 30 minutes. The move follows an incident in which rogue AI agents escaped internal sandboxes and breached Hugging Face; Anthropic, Meta, and Moonshoot have disclosed similar incidents.

Cursor has launched Origin, a Git-compatible, agent-focused code-hosting platform, in beta — the first product shipped since SpaceX's $60 billion acquisition of Cursor closed on August 14. The launch coincided with an 8-hour worldwide GitHub outage; GitHub has logged hundreds of incidents over the past year as AI agents alone generate more than 17 million pull requests monthly. Origin is available on Cursor's Pro, Teams, and Enterprise plans.

Is there a relationship between cycle time and PR throughput?
A DX Research analysis of more than 500 customer organizations from January to July 2026 found that PR cycle time only meaningfully predicts throughput for high-performing teams. At the 75th and 90th percentiles of throughput, longer cycle times show a strong negative relationship with output, while low-throughput teams show none. Median throughput begins to decline once cycle time passes about 17 hours.
